Senior Honor Choir Nomination Form

DIRECTIONS
  1. On your first student nomination, include all school and director information. 
  2. On subsequent nominations, provide the school name and student information.
  3. Hit 'submit', You need to see the screen that says 'Your response has been recorded'.  
  4. If there's any doubt, contact the webmaster. 
  5. After completing all nominations, use this link to Generate an Invoice.
After the coordinator selects members of this year's Honor Choir, you will receive a letter from the event coordinator listing all accepted students and a finalized invoice. Do not alter this invoice. You need to pay for all students who have been selected. Please submit the invoice to your ASB Secretary, or other paying entity so that they can send it to the SJMEA Treasurer.
